Across the United States, maternal and newborn outcomes remain unacceptably poor, with persistent disparities disproportionately impacting Black, Indigenous, immigrant, and rural communities. At the same time, workforce shortages, hospital closures, and fragmented systems are leaving more communities without access to safe, coordinated care.
But this moment also presents an opportunity.
There is growing demand for care that is personalized, risk-appropriate, and delivered close to home—and increasing recognition that we need stronger foundations and better coordination across levels and locations of care to achieve it.
What is primary maternity care?
Primary Maternity Care is not a single program or model—it is an organizing principle for how maternity care systems are designed.
In most high-performing health systems around the world, care is built on a strong foundation of community-based, first-line services that meet the majority of needs across pregnancy, birth, and the postpartum period. In the United States, this concept is still unfamiliar—and largely underdeveloped.
